This questionnaire is for customers with a self-catering property that has met the criteria for business rates. Please only complete this questionnaire once all the criteria have been met.听

You can complete this questionnaire digitally or print it and complete it manually.听

Once complete, save the questionnaire and attach it to an email to鈥�specialist.rating@voa.gov.uk with the subject heading 鈥淪elf-catering application for business rates鈥� and submit.听
