Use this form to ask the First-tier Tribunal (Mental Health) to ask the tribunal for a legal representative or to see the tribunal doctor.

Details
This guidance and form are for anyone who will be having a mental health tribunal soon. If you did not apply for it, it has been arranged because the law requires your case to be reviewed by a tribunal.
