Statutory declaration for changing a child's name by deed poll: Form LOC024
This form must be completed by someone that knows you and the child. Send it with your application to enrol the child's change of name to the Royal Courts of Justice.

Before applying by post you should first read the full guide on changing a child鈥檚 name by deed poll.
You must also complete and include:
- a child deed poll form if applying by post (not needed for online applications)
- an affidavit (statement) of best interest
- photocopies of certain documents 鈥� these will be your 鈥榚xhibits鈥�. See the full guide for information about the documents you must include
